How will my target machine connect to my LHOST? Do I have to be in a LAN connection or can it be on a WAN connection?

you have to be in a LAN or you have to port forward your router

Will any other machines be able to connect to my LHOST? If so, how will I be able to tell which one is my target machine?

If youre using metasploit you can tell it to only accept connections from your target machines public IP and remember if you are going across the internet your lhost is your public IP.
